알레르기성 기도 질환에서 줄기세포 유래 세포외 소포체의 응용

Abstract

Mesenchymal stem cells (MSCs) have been reported to be promising candidates for the treatment of allergic airway diseases. However, MSCs themselves have several problems including immune rejection, risk of aneuploidy, difficulty of handling, and tumorigenicity. An increasing number of studies demonstrated that administration of conditioned media or extracellular vesicles (EVs) released by MSCs is as effective as the MSCs themselves in suppression of allergic airway inflammation. EVs can exert their effects by delivering their contents such as proteins, mRNAs, and microRNAs to recipient cells. Furthermore, the administration of MSCs-derived EVs may reduce potential safety risks associated with stem cell therapy, suggesting that MSCs-derived EVs may be a promising alternative to cell therapy for allergic airway diseases. This review examines the current understanding of the immunomodulatory properties of MSCs-derived EVs and its therapeutic implication for allergic airway diseases.

서 론

줄기세포(stem cell)는 다양한 세포로 분화할 수 있는 분화능(differentiation capacity)과 지속적인 자가 분열능(self-renewal)을 가지고 있어 손상된 조직을 재생할 수 있는 능력을 가지고 있을 뿐 아니라 면역 체계를 구성하는 다양한 면역 세포의 기능을 조절할 수 있는 능력을 가지고 있다고 보고되었다 [1]. 줄기 세포의 이러한 특징 때문에 임상 질환의 다양한 부분에서 장기 및 조직의 손상에 대한 기능 회복을 목표로 활발한 연구가 진행되고 있으며 [2,3], 줄기 세포의 발달로 인류는 새로운 양상의 의료 형태를 꿈꾸게 되었다. 세포를 주입하면 그것이 ‘조직 손상이 있거나 염증이 있는 부위를 스스로 찾아가 치료 효과를 나타낸다’고 하는 세포치료를 경험하게 된 것이다 [4,5]. 특히 성체줄기세포(adult stem cell)인 중간엽줄기세포(mesenchymal stem cell)의 경우는 이미 몸 안에 있던 세포를 주입하는 것이기 때문에 배아줄기세포(embryonic stem cell)에 비해 종양의 발생 가능성이 적고 윤리적인 문제를 피할 수 있어 다양한 질환에 세포치료로 활발히 연구되고 있다.
중간엽줄기세포는 다능성(multipotent) 미분화 성체줄기세포로 대부분의 신체 조직에서 발견되며, 연골모세포(chondroblast), 골모세포(osteoblast), 지방세포(adipocyte), 골격세포, 심근세포, 신경세포 등을 포함하는 다양한 형태의 세포로 분화할 수 있어 줄기세포를 이용한 연구에서 가장 많이 사용되고 있으며 [6-9], 조직의 항상성을 위한 미세환경(microenvironment)의 유지와 손상된 조직의 재생 및 리모델링에 중요한 역할을 한다고 알려져 있다 [6-9]. 또한 중간엽줄기세포는 T세포, B세포, NK세포의 기능과 증식을 억제하고 수지상 세포(dendritic cell)의 활동을 조절하며, 조절 T세포(regulatory T cell)의 반응을 유도하고 발현을 증가하는 역할을 한다 [10,11]. 중간엽줄기세포의 이러한 면역 조절 능력은 자가면역성 뇌척수염증(autoimmune encephalomyelitis), 염증성장질환(inflammatory bowel disease), 관절염(arthritis), 패혈증(sepsis), 이식편대숙주병(graft-versus-host disease), 다발성 경화증(multiple sclerosis) 및 제1형 당뇨병(type I diabetes)을 비롯한 다양한 면역 관련 장애의 치료를 위해 연구되었으며 [12-18], 최근에는 알레르기성 기도 질환인 알레르기 비염과 기관지 천식에서도 줄기세포의 면역 치료 효과가 증명되었다 [19-25].
하지만 중간엽줄기세포를 이용한 치료법은 극복해야 할 몇 가지 중요한 한계가 있다. 먼저 인체에서 유래된 생체물질(living material)로써 치료 용도로 사용하기 위한 질적 관리(quality control)에 어려움이 있다. 또한 중간엽줄기세포의 분리 절차, 배양 조건, 저장 방법 및 투여는 세포의 생존력 및 효능에 큰 영향을 미치기 때문에 이로 인한 높은 비용과 낮은 재현성이 문제가 된다 [26,27]. 세포를 이용한 치료로써 면역 거부 반응, 종양 형성과 같은 예기치 않은 안정성 문제를 예방하기 위해 엄격한 모니터링이 필수적이고, 장기적인 관점에서 치료의 안정성에도 한계가 있다 [26,27]. 그리고 임상적으로 실제 표적 장기에 도달하는 세포의 양을 정확히 알 수 없기 때문에 투여하는 용량을 산출하기도 매우 어렵다.
최근 줄기세포에 의한 치료 효과는 줄기세포 자체뿐 아니라 줄기세포에서 분비되는 여러 물질들이 중요한 역할을 한다고 보고 되고 있어 줄기세포 배양액에서 분리되는 여러물질에 대해 많은 연구가 이루어지고 있다. 중간엽줄기세포가 병변 부위에 노출되었을 때 미세환경(microenvironment)에 반응하여 다양한 치료 물질을 분비하며, 중간엽줄기세포가 분비하는 다양한 물질들이 병변 부위를 회복시키는 작용을 줄기세포의 주변 영양효과(paracrine effect)라고 한다 [4]. 중간엽줄기세포에서 자가분비유도(autocrine induction) 또는 주변분비유도(paracrine induction)로 분비되는 물질들은 혈관생성인자(angiogenic factors), 성장 그리고 영양 인자(growth and trophic factors), 케모카인(chemokines), 전염증성 사이토카인(pro-inflammatory cytokine), 항염증 사이토카인(anti-inflammatory cytokines), 세포외 소포체(extracellular vesicles)를 포함하고 있다 [4]. 이에 본 연구에서는 현재 알레르기성 기도 질환에서 줄기세포로부터 유래한 세포외 소포체를 이용한 대표적인 연구 결과들을 소개하고 임상적인 적용에 대한 가능성에 알아보고자 한다.

본 론

줄기세포 유래 세포외 소포체의 특징

세포외 소포체는 세포에서 세포 외 환경으로 다양한 형태로 분비되는 지질 이중측 막에 싸여 있는 구형의 물질을 의미하며 조혈세포(hematopoietic cell), 내피세포(endothelial cell), 상피세포(epithelial cell), 암세포(cancer cell) 등과 같은 다양한 세포로부터 분비된다. 세포외 소포체는 각각의 크기, 모양, 생체생성, 기원 및 조성에 따라 30~100 nm의 exosome, 100~1,000 nm의 microvesicles, 그리고 1~5 μm의 apoptotic body로 분류된다(Fig. 1) [28]. Exosome은 대개 크기가 균질하고 endosomal system에서 세포외배출(exocytosis)을 통해 생성되며, microvesicles은 크기가 비교적 이질적이고 원형질막(plasma membrane)이 바깥으로 솟아나와 분리되어 방출된다(Fig. 2) [28-30]. 세포로부터 방출된 세포외 소포체는 다양한 단백질, 지질, 유전체 등의 다양한 생체신호전달물질을 포함하고 있으며, 세포-세포 의사소통 및 주변 미세환경과의 상호작용에 중요한 역할을 수행한다 [30-32]. 세포외 소포체가 분비되어 표적세포로 이동하여 다양한 치료인자들을 전달하는 기전은 세포외 소포체가 분비되어 표적세포의 원형질막에 접촉이 되면 세포외 소포체와 원형질막의 상호작용[직접적인 용해(direct fusion) 또는 세포내섭취(endocytosis)]을 통해 흡수된 다음 다중 소포 엔도좀(multivesicular endosome)에 도달하고, 대부분의 경우 라이소좀으로 이동하여 세포외 소포체에 있는 단백질과 지질이 분해되어 표적세포에 대사물질을 공급하게 된다(Fig. 3) [30].
성체줄기세포 이식 치료의 기전이 질환 부위로 이동한 줄기세포의 치료인자 분비 효과에 의한 것으로 보고되면서, 줄기세포 응용 치료제 분야에서 줄기세포 유래 세포외 소포체의 중요성이 강조되고 있다. 줄기세포 유래 세포외 소포체는 줄기세포로부터 분비되는 다양한 치료인자(단백질, microRNAs, mRNAs 등)들을 함유하고 있으며, 줄기세포 세포막에 위치하는 각종 수용체들을 포함하는 이중 지질막에 싸여 있어 혈액 내 순환 중 분해효소 및 분해화학물질로부터 내부 봉입 인자가 보호됨과 동시에, 병변세포 또는 미세환경으로 표적화가 가능하다. 세포외 소포체는 리포좀(liposome)과 유사한 생물학적 구조로 인해 중간엽줄기세포보다 생체 내에서 보다 안정적이며, 약물 전달의 매개체나 치료적 물질로 응용하기 위해 세포외 소포체의 함량 및 표면 특성을 개선하는 것이 상대적으로 쉽다 [33,34]. 또한 무세포 치료(cell-free therapy)이기 때문에 저장 및 취급 절차가 중간엽줄기세포와 비교하여 비용 효과적이며 종양의 발생 가능성, 면역원성(immunogenicity), 종양원성(tumerogenicity)과 같은 안정성 문제로부터 자유롭다 [33,34]. 이미 줄기세포 유래 세포외 소포체는 줄기세포 치료 효능의 주요 기전 중 하나로서 줄기세포와 동등 혹은 그 이상의 치료 효능을 가진다는 여러 연구결과들이 보고되고 있으며 [35-37], 현재 미국, 유럽 등에서 줄기세포 유래 세포외 소포체 치료제의 안정성 확보를 위한 초기 단계의 임상연구가 진행 중이다. 줄기세포 유래 세포외 소포체의 우수한 치료 효과가 점차 밝혀짐에 따라, 현재 줄기세포 치료분야의 한계를 극복할 수 있는 이상적인 대안으로서 그 중요성이 강조되고 있다.

알레르기 기도 질환에서 줄기세포 유래 세포외 소포체의 효과

알레르기 비염은 천식과 함께 대표적인 아토피성 질환으로 알레르기 비염 환자의 30%가 천식을, 천식 환자의 70%에서 알레르기 비염을 동반할 정도로 서로 밀접하게 연관되어 있어 “one airway, one disease”라고 할 수 있다 [38,39]. 알레르기 비염과 천식은 다양한 원인 항원과 이에 대한 특이 면역글로불린 E에 의해 발생하며, 면역학적으로 호산구의 유출과 Th2 세포의 활성화를 특징으로 한다. 즉 알레르겐에 특이적인 Th2 면역반응으로 Th2 세포에서 분비되는 IL-4, IL-5, IL-13과 같은 사이토카인에 의한 만성 호산구성 염증이다 [40]. 최근에는 알레르기성 기도 질환의 병인에서 Th1/Th2 반응의 불균형뿐 아니라 조절 T세포의 불완전한 억제가 과도한 Th2 반응에 중요한 역할을 한다고 보고되었다 [41,42].
중간엽줄기세포는 면역조절효과가 있는 수지상 세포의 증식과 기능을 억제하고, T세포와 B세포로 분화할 수 있어 알레르기 기도 질환의 새로운 치료법으로 주목을 받았다. 동물모델을 이용한 여러 연구에서 정맥으로 주사한 중간엽줄기 세포에 의해 알레르기 마우스 모델의 코와 폐 조직에서 호산구의 침윤이 유의하게 감소되었으며, 기도 과민성과 알레르기 증상이 호전되었다. 또한 알레르겐 유발 후 기관지 폐포 세척액에서 증가한 IL-4, IL-5, IL-13와 같은 Th2 사이토카인, 면역글로불린 E를 감소시키며, 조절 사이토카인과 조절 T세포의 증가를 유도하였다고 보고하였다 [19,22-25,43-45]. 이에 대한 기전으로 기도나 정맥으로 주입된 줄기세포가 폐로 이동하여 IDO, TGF-β, PGE2의 분비를 촉진하고 이는 조절 T세포의 증가와 IL-4 양성 T세포의 감소, 그리고 IFN-γ 양성 T세포를 증가시켜 결과적으로 폐 조직에서 호산구의 침윤과 IL-4, IL-5, IL-13과 같은 Th2 사이토카인 그리고 알레르겐 특이 면역글로불린 E와 G1을 감소시키고, Th1 사이토카인과 IL-10, TGF-β와 같은 조절 사이토카인을 증가시킨다고 알려져 있다(Fig. 4) [25]. 이러한 중간엽줄기세포의 면역 조절작용에 대한 기전에 대해서 많은 연구들이 시행되었으며 줄기세포에서 분비되는 면역 조절작용을 가지고 있는 prostaglandin E2(PGE2), transforming growth factor(TGF)-β, IL-10과 같은 여러 가용성 인자(soluble factor)들이 관여함을 알 수 있었다(Table 1) [4,46-49].
최근에 줄기세포 배양액(conditioned media)과 줄기세포 배양액에 포함되어 있는 세포외 소포체가 알레르기성 기도질환에서 줄기세포 자체와 동등한 면역 조절 효과를 보인다고 보고되었다 [50]. 정맥으로 주사한 줄기세포 배양액과 세포 외 소포체는 줄기세포와 동일하게 천식 마우스 모델에서 기도 과민성과 폐 조직에서 호산구의 침윤을 유의하게 감소시켰으며, 기관지 폐포 세척액에서 알레르겐 유발 후 증가된 IL-4, IL-5, IL-6, IL-17, RANTES의 수치를 의미있게 감소시켰다. 림프구혼합배양(mixed lymphocyte culture)에서 IL-4, IL-5, IL-17은 유의하게 감소되었으며, IFN-γ은 유의하게 증가되었다. 인체 지방 조직 유래 중간엽줄기세포와 세포외 소포체의 정맥 투여는 모두 폐 조직에서 콜라겐 섬유의 침착, 기관지 폐포 세척액에서 전체 염증 세포 수와 호산구의 비율, 폐 조직에서 IL-5의 수치, 흉선에서 CD3+CD4+ T세포의 감소에 동일한 효과를 보였지만 폐 조직에서 호산구의 수, IL-4와 IL-13, eotaxin의 수치, 기관지 폐포 세척액에서 CD3+CD4+ T세포, 폐 기능에는 다른 효과를 보였다 [51]. 마우스 지방 줄기세포 유래 세포외 소포체의 면역조절 기전에 대한 in vitro 실험에서 세포외 소포체는 미성숙 수지상 세포의 CD40, CD80, 그리고 CD86의 발현과 arginase1, CCL22, IL-10, TGF-β와 같은 M2 macrophage 표지자의 발현을 증가시킴으로써 eotaxin과 IL-10의 발현 감소, TGF-β와 IL-10의 발현 증가를 통해 폐상피세포의 Th2 매개 염증을 감소시켰다 [52]. 그러나 줄기세포와 다른 줄기세포 유래 세포외 소포 체의 알레르기성 기도 질환에 대한 면역조절효과의 기전에 대해서는 추가적인 연구가 필요할 것으로 생각된다.

줄기세포 유래 세포외 소포체의 한계점과 앞으로의 전망

세포외 소포체에 대한 많은 연구들이 진행되고 있고 면역조절 치료제로서의 효과에 대해서도 많은 보고가 이루어 지고 있지만, 실제 환자에게 치료적으로 사용을 하려면 여전히 많은 제약과 한계점이 존재한다. 세포외 소포체를 임상적으로 적용하려면 다음과 같은 문제점을 고려해야 한다. 먼저 충분한 효과를 보장할 고농도의 대규모 중간엽줄기세포 유래 세포외 소포체를 확보하는 문제이다. 세포외 소포체는 중간엽줄기세포의 배양배지로부터 분리되기 때문에 줄기세포의 수, 배지의 부피와 세포외 소포체를 분리하는 시기를 포함한 배양 조건은 세포외 소포체의 양 뿐만 아니라 질적인 부분 모두에서 영향을 미칠 수 있으므로 매우 중요하다 [53]. 또한 중간엽줄기세포 유래 세포외 소포체를 배양배지에서 분리하는 여러 방법들이 소개되고 있지만 이러한 다양한 방법들에 대한 최적화가 선행되어야 하며 과정에 대한 적절한 규제 또한 필요하다 [53].
다양한 질병에 대한 중간엽줄기세포유래 세포외 소포체의 안정성과 효능에 대한 임상 평가가 시행되어야 하고 이론적인 결과를 뒷받침 해줄 수 있는 정확한 작용 기전에 대한 연구가 시행되어야 한다. 줄기세포 유래 세포외 소포체의 치료 효과와 관련이 있는 과 발현된 유전자나 단백질의 집합을 확인하는 방법인 functional enrichment of gene analysis, microRNA expression patterns에 대한 분석 또한 중요하며, 이미 몇몇 연구들에서는 특정 유전자의 발현 패턴과 차등 발현의 특성에 대해 보고 하였다 [54,55]. 중간엽줄기세포 유래 세포외 소포체의 치료 효과와 연관이 있다고 알려진 많은 물질들 중에 paraoxonase-1(PON1)은 천식 마우스 모델에서 폐의 호산구 염증과 기관지 벽의 두께, 배상 세포(goblet cell)의 증식을 유의하게 감소시켰으며, 기관지 폐포 세척액에서 염증세포와 IL-4, IL-5, IL-13, TNF-α와 같은 염증성 사이토카인과 말초 혈액에서 알레르겐 특이 면역글로불린 E와 G1을 유의하게 감소시켰다. 이러한 결과는 PON1이 천식 마우스 모델에서 기도 재형성과 기도 염증을 감소시킬 수 있다는 것을 의미하며, 중간엽줄기세포 유래 세포외 소포체의 알레르기성 기도 질환에 대한 면역조절효과의 기전에도 연관이 있을 가능성이 높다고 생각된다.

결 론

현재 다양한 질환에서 줄기세포 유래 세포외 소포체의 임상적용을 실현하고자 하는 노력이 국내외 지속적으로 증가하고 있다. 알레르기성 기도 질환에서 줄기세포 유래 세포외 소포체에 대한 연구는 아직 초기 단계에 있지만 지금까지의 연구 결과를 통해 세포외 소포체는 알레르기성 기도 질환에서 줄기세포와 동일한 면역조절효과를 보이고 새로운 치료제로서의 가능성을 보여주었다. 줄기세포에서 한계점으로 제시된 저장 및 유지와 관련된 안정성, 재현성 및 비용 효율성을 극복할 수 있다는 측면에서 줄기세포 유래 세포외 소포체는 미래에 유망한 치료 물질일 수 있다는데 의심의 여지가 없다. 하지만 줄기세포 유래 세포외 소포체의 치료 효과와 관련이 있는 유전자와 단백질을 밝혀내고, 다양한 in vitro/in vivo 질환 모델에서 치료 기전에 대한 과학적 근거 확보가 필요하다. 또한 줄기세포 유래 세포외 소포체를 치료적 물질로 사용하기 위한 표준화와 최적화에 대한 확립도 선행되어야 할 것이다. 알레르기성 기도 질환에서 무한한 가능성을 가진 줄기세포 유래 세포외 소포체의 임상 적용 및 상용화가 실현된다면 현재 제한된 줄기세포 응용 치료법의 임상적용 범위가 크게 확장될 수 있을 것으로 생각되며, 이를 통해 근본적인 치료가 되지 않는 알레르기성 기도 질환에서 치료 방향의 패러다임 자체를 바꿀 수 있을 것으로 생각된다.
